Who qualifies in Pennsylvania?
Pennsylvania residents need an approved serious medical condition certified by a participating physician. Examples include anxiety disorders, PTSD, cancer, and severe chronic or intractable pain.

The state fee is due once every 12 months, independently of card issuance. Pennsylvania sends payment instructions 30 days before it is due.
